DETERMINING YOUR WELLNESS PROFILE: THE MEDICAL EVALUATION - THE NEED FOR A MEDICAL EVALUATION
Most physicians indicate that a physical examination is necessary for individuals over the age of 40, those with symptoms of heart disease or other medical ailments, and those who have previously been sedentary. Some physicians favor a comprehensive exam; others prefer only general screening.
The recommendations of the American College of Sports Medicine (ACSM) provide sound information related to the health, status, and age of the participant (see Table 2.1). They classify individuals who may undergo exercise testing into three categories:
1. Apparently healthy Those who appear to be in good health and have no major coronary risk factors.
2. Individuals at higher risk Those who have symptoms suggestive of heart disease, pulmonary or metabolic disease, or at least one major coronary risk factor.
3. Individuals with disease Those with known cardiac, pulmonary, or metabolic diseases.
The National Heart, Lung and Blood Institute (NHLBI) advises that most people under 60 years of age do not need a medical examination prior to beginning a gradual and sensible exercise program. Their rationale is based on the realization that sedentary living is a far more dangerous practice than exercising without a physician's approval, that many people will not take the time to secure an examination, and that a recommended medical exam is nothing more than another excuse to avoid exercise.
Certainly it is desirable for everyone to have a complete medical examination before their physical fitness evaluation and the start of a new exercise program to increase safety and aid in the exercise prescription. It is also a generally accepted fact that certain categories of people face some risk when engaging in fitness programs without a medical examination.
